AL Team Matchups:
New York Yankees: AL East Champs
vs.
Detroit Tigers: AL Central Champs (or Minnesota Twins...although it's very unlikely)
Los Angeles Angels of Anaheim (<worst name in MLB): AL East champs
vs.
Boston Red Sox: AL Wild Card Winner
NL Team Matchups:
Los Angeles Dodgers: NL West Champs (<Not necessarily true, but by no means a s t r e t c h)
vs.
Either the St. Louis Cardinals or the Philadelphia Phillies
The remainder of the above 2 teams (Cardinals or Phillies)
vs.
Colorado Rockies: NL Wild Card winner
Okay, so I made a few assumptions, but there are going to have to be some radical streaks for any of these likely matchups to change. Other options:
Detroit Tigers go 1-3 or worse in their remaining 4 games AND Minnesota Twins go 3-1 or better in their remaining 4 games. If BOTH of these events occur, then there will either be a 1 game playoff (in the event of a tie), or the Minnesota Twins will win the division outright (yeah, right!).
Otherwise, the aforementioned teams are all going to make the playoffs. The only thing pending is the matchups in the NL.
PS: It would be bad for MLB if any of the ReD teams won the crown...
